Our lab is updating the genital culture. We are debating to get rid of the gram stain. Should the gram stain be ordered for the diagnosis of Bacterial Vaginosis? Or should BV be idenitified from the plate?

We do a few genital cultures still, and the physician is good about including the diagnosis. But I've been concerned about the situation in which the genital smear suggests bacterial vaginosis (no Lacto, many small variable rods and rare yeast), but then the culture grows many yeast. We then feel obligated to ID the yeast, but actually the gram stain suggested yeast was a minor presence. When the gram stain shows rare yeast, should we limit our yeast workup? Possibly report as "Many yeast, no further ID indicated by original smear." In this case, the dx was polycystic ovaries. Thanks.

How should a negative (no pathogens) genital culture report (not a GBS screen) be worded? Can you include the statement "No Gp B Beta strep isolated" if you did not set up a LIM broth on these cultures? Should LIM broths be set up on all genital cultures if physicians sometimes order a genital culture instead of a GBS screen?

We follow the guidelines for Group B Strep screenings for pregnant patients using LIM broth. We only test beta hemolytic colonies for Group B. Recently we had a culture that the tech tested because he thought the colonies looked suspicious. They were indeed a nonhemolytic Group B. We are now concerned that we could be missing these as per our protocol we only ID hemolytic streps. Is this a real concern and if it it, is there a cost effective way to test for nonhemolytic Group B?

In your opinion, what should be worked up in genital cultures during pregnancy ? If the diagnosis is just pregnancy, shouldn't they just be ordering the beta strep B screen? We have a couple physicians that screen for E.coli. Is this appropriate? Thank you

I'm a veterinarian, submitting vaginal smears to a lab, for bacterial culture and sensitivity.What we usually do in the clinic is we put the swab in a sterile tube with nutrient agar. But just recently the lab gave me chocalate agar media. They told me to directly put my specimen in it.. I want to know the significance of this chococalate agar in culturing samples coming from the genital area. And can't samples from vaginal smears grow in regular nutrint agar?

For genital cultures, we are wondering if it would be appropriate to bill for a Gardnerella screen (code 87081) along with code 87070. We are currently using five plates for our genital cultures: Chocolate, MacConkeys, Blood, GC-Lect, and HBT-Bilayer to check for Gardnerella. The HBT-Bilayer is quite expensive, and we are wondering if reimbursement for this is a possibility. Thanks.

My question is concerning screening for Group B strep on pregnant women, CDC
recommends incoulating a plated media and also to include a broth media such
as LIM broth or Todd Hewitt with antibiotics. If the originial plated media
from the specimen does not show evidence of Group B strep and is not
overgrown with gram negative rods, just other usual flora - enterococci,
Coagluase neg. staph., lactobacilli, and the like, is it necessary to
subculture the broth media to plated media? If that subculture then has
Group B strep, is this low number of strep significant enough to cause
streptococcal infection in newborns?
