What are the advantages or disadvantages to the use of the "LEAN" process in the Microbiology Laboratory? Thank you for any insight you can provide.

Currently we are planing to make a cut off time for proscessing our specimen at 0600 pm so any specimem arrive after 0600 pm will be read with the follwing day specimen .Please let me know if that is okay.my staff issue that some of our plates might have some growth at 12 hrs & why they will be read the following day,but that is the only way to make the system work if we make a cut off point other wise we will be working a round the clock ,please help in making the desicion on this issue .

Hello, I am interested in purchasing a CDC-rom or DVD showing how to properll collect specimens for culture - sputums, urines, nasopharyngeal swabs, etc...It one thing to read on how to collect, but it is another to see how to collect specimens. Are there any "benchmarks" for the number of cultures read by a technologist in an 8 hour shift? For example: Routine stools Respiratory cultures Wound Urines Anaerobes Positive Blood Is it reasonable to expect 1 tech on each of the following benches to read 225 wound cultures, 300 urines or 140 positive blood workups?

IN THE LAB, LABORATORY TECHNOLOGISTS AND MEDICAL TECHNOLOGISTS HAVE DIFFERENT LICENSURES THROUGH THE STATE BASED ON THE LEVEL OF EDUCATION AND COMPLEXITY OF TESTING PERFORMED. We are being asked to place our Microbiology Procedures into CLSI Laboratory Technical Procedures Manuals; Approved Guideline - Fourth Edition GP2-A4, which includes a strange new phenomenon the if..........then...........scenario. Page 26 of the guide shows a sample urine procedure with the following: if there is then no growth reinc, enter prelim orgs 1-3 follow instr appd 1 iD/susc orgs 4 or > report as..... This is a brief piece of the chart. This example was submitted by the Microbiology Technical Work Group, Sutter Health Laboratory Integration Project, Sacramento, CA. Do you know of anyone in the United States that has placed their Microbiology Procedures into this if...then.... format? If not, why not? Where did the If....... then....... format arise? We currently bill CPT code 87081 for BSA and MSA screening plates in addition to 87070 for CF sputum cultures. We also include BSA and MSA plates on Quant. bronchial cultures on CF patients. When we attempted to bill CPT code 87081 for these screening plates in addition to 87071 (quant. culture), we received a denial. We added the -59 modifier to these 87081 codes. Are we coding correctly?

My question has to do with the educational and certification requirements that qualify someone to be able to read gram stains. Our evening staff has recently hired someone who has a B.S. in Biology that they would like us to train to read gram stains. Under CLIA guidelines is this person qualified to read gram stains?

With the exception of GC screens, we do not hold most of our cultures a full 48 hours. Urines plated before 3pm are finalized the next day if sterile or contaminated. Campylobacter plates are also not held a full 48 hours as well as most wounds, fluids and respiratory specimens. Is this considered less than good patient care?

In our hospital, PPD's are administered by the Microbiology staff and then read by the patient's nurse. Is there any stipulation as to who is qualified to administer a PPD? We are under the impression that lab technologists or phlebotomists may not have the licensing or competency evaluations needed to give "injectables". Please comment.

I am wondering if there are any benchmarks available for clinical microbiologists that would provide insight into the number of cultures a bench technologist can "comfortably" review in a day's shift without sacrificing accuracy. In other words, is there a limit similar to that of a cytologist at which point additional help (man-power) is necessary? We are in need of adding additional help in our microbiology department and our administrators are impressed with benchmarks but I haven't seen anything for micro in the journals lately. Please let me know if you are aware of anything.

Are there any workload volume guidelines for Microbiology laboratories/personnel? I know that there are Federal and State regulations, designed to ensure the reliability of Pap smear screenings, i.e., restrictions on the number of smears which may be read. The CAP discontinued its workload units in 1992.
