We want to switch from PVA to Protofix for ova and parasite collection. What validation is required? Do we need to collect samples from patients in duplicate? If so, how many positives/negatives? Is there another way to validate without involving patients?

Would you give us directions for a trichrome method of staining Protozoa that works well using PVA with zinc fixative? Since giving up the mercury fixative, we have trouble getting good nuclear staining.

Regarding the test Ova and Parasites, since both a concentration and a permanent smear (in our case Trichrome) is required by the College of American Pathologists, can we use both the CPT Code 87177 (concentration) AND 88313 (Trichrome) for each specimen? Thank you!

Does PVA fixative render fecal material noninfectious? Our hazardous waste disposal company is requiring us to certify that the PVA vials used for parasitology are noninfectious and state the disinfectant (Schaudinn's fixative, perhaps?) before they will pick them up and dispose of them.

I have a question about a previous answer relating to CPT coding of parasite exams that was answered on 08/03/04. It was indicated to use 88312 as the new correct code for the trichrome. We recently found only Giardia trophs on an O&P exam, and the Merifluor Giardia antigen was negative. This made us question the protocol to "screen" for Giardia first before ordering a full O&P when trophs will be missed by this assay. Should we include a disclaimer of some sort on our reports?

We have recently become CAP accredited and are required to perform trichrome stains on all O&Ps. We had previously performed them only if the physician requested "look for amoebae" or if the technologist saw something suspicious on the direct or concentrated wet mount. We are being denied reimbursement for the CPT code 88313 which is specifically stated on page 277 of the 2003 CPT Code Book as the correct code to use. The lack of reimbursement was not a great concern when we performed the test infrequently, but due to the significantly increased workload, we would like to be able to recover the cost. Can you suggest an alternative code that is acceptable to Medicare or is there a way to state a policy acceptable to CAP that will keep us from having to do this test on all specimens? After adding this test and looking at more than 500 slides, we have yet to find one that was not indicated by what was seen on the wet mounts.

I am a student studying in London in tropical diseases and have heard about a type of nematode parasite that resmebles hairs,fibres,eyelashes and crystals depending on where it is on the human body and what metamorphic stage they are at. This is called unidentified skin parasite infection and there is only three pages in total on the internet on it. Can it be transferred by breathing ie airborne, does it kill the human host and if so how long, I heard that someone went deaf from it, is that possible? What are the best treatments of slowing down the effects. Did it really stem from being a parasite on a scabies or crab lice and if so how does it get so big? How can there be no treatment?

Our facility performs many O&P exams with very few positve results. We also have an immunoassay for Cryptosporidium and Giardia. I believe those are the most likely parasites to be found in the southeast and that we should be screening with the immunoassay instead of performing the ova and parasite examination. I would like to reserve the ova and parasite examination for when a specific parasite outside of Giardia or Crypto is suspected and/or when the patient has a relevant travel history. Any thoughts on the subject. Does anyone know of a particular article(s) that refer to this subject.
