NCASM Spring 2012 Conference On-line registration

Instructions: You must carry out both Steps 1 & 2 to complete the registration process:

Step 1: Complete the registration form below (10 quick questions) then click the "Submit" button after question #10.
After you "Submit" the completed form, proceed to Step 2 to pay through PayPal.

Step 2: Use the "Buy Now" buttons at the bottom of the page to send payment through PayPal (accepts credit cards also).
Note: You do NOT need a PayPal account to pay with your credit card.

Payment Instuctions:

1. Fees are listed below from two pulldown windows: One for "Professionals (non-students)" and one for "College Students". Select the correct payment category from the appropriate pulldown window below.

2. Click the "Buy Now" button, which will link you to PayPal. Note: You don't have to set up a PayPal account. You can just click continue after "Use your credit card or bank account."

3. On the page "Review your payment": If the name on the credit card is NOT the person who will be attending the meeting, click "Add special instructions to merchant", and type the Name of the person who will be attending the meeting.

4. Once you have completed your payment, you will receive an email from PayPal confirming your payment. If you do not receive email confirmation, then payment was not completed. The email from PayPal will act as your payment receipt. Please bring the email receipt to the meeting.
