Preregistration Deadline: December 12, 2001
The preregistration deadline is December 12, 2001. After that date,
registration fees will increase by $100 per category ($50 per student category). Please
note that registrations will not be processed until payment is received. A confirmation
letter and receipt of payment will be sent to the address provided on the registration
form. Please allow 2-4 weeks for your registration to be processed.
Registration Categories and Fees
The registration fee includes 4 lunches, 1 dinner banquet,
3 receptions, and coffee breaks. The categories are as follows:
ASM Member $425
ASM Member Student (Predoctoral)* $295
Nonmember $525
Nonmember Student (Predoctoral)* $345
*Students must include a legible photocopy of a
university ID validated for spring 2002 as proof of their predoctoral status.
Member Rates
To register at the lower ASM member rate, you must be logged-on as an ASM
member from the ASM home page and then use the link to the ASM Member Registration Form
below.
Non-members can join ASM before they register and receive the lower rate. If you choose
this option, you will not be able to use online registration until you receive
notification that your ASM membership has been processed and you have your ASM membership
number with which you can log onto the ASM site as a member from the home page. For an ASM

Cancellation Policy
All registration cancellations must be in writing and received by ASM
no later than December 26, 2001. A refund of your registration fee minus a $50
administrative fee will be returned for cancellations received by December 26, 2001. No
refunds will be granted after that date.
Online Registration
Online registration is now available for this ASM Conference. Please read
through all of the following instructions before starting the online registration process.
It is recommended that you print these instructions to have available during the online
registration process. While registering, all required information will be indicated with
an asterik (*). If you do not complete required information, you will not be allowed to
proceed through the registration process until you have done so.

A. Personal Contact Information
Please complete all information indicated with an asterik (*) in this section and click
on the "Update" button. (If you are an ASM member, this information should be
automatically supplied.) You will be asked to review this information once. If you
need to change any information, click on "Personal Contact Information is NOT
correct" to return to the previous page. If all of your information is correct, click
on
"Personal Contact Information is correct" to proceed to the next section.
NOTE: Your registration confirmation and receipt will be
mailed to the address provided in this section. This information is used to generate your
name badge and is included in a participant list distributed to all conference attendees,
so please provide your organization and your business address. It is also important
that you include and double-check the accuracy of your e-mail address, as much of the
conference information is communicated in this manner.
B. Demographic Information
Please complete all three items in this section by indicating only one choice and click
on the "Update" button. You will be asked to review this information once. If
you
need to change any information, click on "Demographic Information in NOT
Correct"
to return to the previous page. If all of your information is correct, click on
"Demographic Information IS Correct" to proceed to the next section.
C. Registration Category and Fees
Item I. At least one selection (either regular or student member) is required to continue
the registration conference. Conference Registration includes access to all sessions,
posters conference materials, and all group meals.
Item II. Late Registration Fee is required of applications postmarked, received online or
by fax after the preregistration deadline of December 12, 2001.
D. Payment Method
You may choose one of two payment methods - 1) Online Credit Card Payment or
2) Alternate Payment Method (e.g. checks or credit cards via mail or fax). Click on
the appropriate link to proceed. Please note that once you make your choice by
clicking on a link, you will NOT be able to change your payment method.
If paying online by credit card, you will need to indicate the type of credit card, the
credit card number, name on the credit card and the expiration date. When
complete, click on the "Payment" button to process. When processing is complete,
you will receive a confirmation page which you can print for your records.
If using the alternate payment method, you will be taken to a summary page titled,
"Alternate Payment Method." You must print this page and complete the check or
credit card information (i.e. credit card type, card number, expiration date and name
as it appears on card used). If paying by check, you must enclose a check for the
total amount, made payable to the American Society for Microbiology, and mail it
with a copy of the form to the address indicated on the form. If paying by credit
card, you may either fax or mail the completed form to the number or address
indicated on the form.
