Preregistration for the 103rd General Meeting is closed. Although we cannot continue to accept
preregistrations, the cost for on-site registration is equal to the cost for full preregistration.

KEY REGISTRATION ELEMENTS
- 1. Name and Address Information
Please provide your full name and address on the form as the successful receipt of General Meeting materials
is dependent on the accuracy of this information. ASM membership numbers must be included for registrants
to be eligible for member rates.

- 2. Demographic Information
Kindly complete the demographic information. For scientific specialty, please choose a discipline
from the list below.

- 3. Hotel Reservations
Hotel reservations are only available to registered General Meeting attendees. Hotels
will not accept direct reservations for rooms. To reserve a room, complete all of section three of the
Registration and Housing Form when you submit your registration. In
addition, please list your roommates, if any, and indicate whether cost, location, or a
particular hotel chain is most important. All reservations require a
$180 deposit per
room.

- It is to your advantage to register promptly, as hotel rooms are assigned on a first-come,
first-served basis. If your requested hotel is no longer available, we will make every
effort to honor your indicated preference for cost, location, or other consideration.

- 4. Registration Fees
Select your registration category, and any optional fees such as Continuing Education service fees or
international courier fee. Please make sure to check off the fees that apply.

- 5. Total and Payment Type
Payment must accompany your registration form, either by check (made payable to ASM in US Dollars, drawn
on a US bank) or with a credit card number. To use fax, phone or website registration, a credit card number must
be provided. ASM accepts MasterCard, Visa, American Express and Discover. Please note that your credit card will be
billed immediately for registration, optional fees and housing
deposit.

- Criteria for Member Rates

- ASM membership numbers must be included in section one for registrants to be eligible
for member rates. Membership numbers are verified by ExpoExchange. ASM reserves the
right to charge the higher non-member rates without notification if a member number is not
included or cannot be verified. For membership verification, please contact the Membership
Department (202-942-9319) or visit the membership section of the ASM web site at www.asmusa.org.

- Criteria for Student and Postdoctoral Rate

- Registrants requesting student and postdoctoral fellow fees must provide either a
legible photocopy of a current University ID or a letter on University letterhead from the
department chair, indicating your status and anticipated graduation date. Only predoctoral
students are eligible for student rate.

- PAYMENT

- Payment must accompany your Registration and Housing Form, either by check (in

- Those registrants canceling between March 14, 2003, and May 16,
2003 will
receive a refund of 50% of the registration fee. You may keep the Final
Program, the Exhibits Guide and, when ordered, the Abstracts Book.
To complete your cancellation, you must return your
badge, Expocard, and confirmation letter. THESE DO NOT NEED TO BE INCLUDED WITH YOUR
CANCELLATION NOTICE. You may mail your badge, Expocard, and confirmation letter under
separate cover after you receive them. Faxed copies of the confirmation letter and badge
will not be accepted. Refunds will be processed after the meeting.
- After May 16, 2003, no refunds will be allowed.
- Optional Fees
- Fees for Continuing Education, Abstracts on CD-ROM, Abstracts
Book, Guests, Federal Express, and the 5K race, are non-refundable.

- Housing
- Please contact ExpoExchange on or before May 3, 2002, should you need to cancel
your room reservation. Individuals canceling on or before April 18,
2003, will be
refunded the full room deposit by ExpoExchange.

- Hotel cancellations made after April 25, 2003 and 72 hours prior
to arrival must be made directly with the
hotel. The hotel will issue your refund. Please allow 30 to 60 days for
processing. Remember to record the date and time you called the hotel, the name of the person you
spoke to and your cancellation number. Failure to cancel your reservation with the hotel
72 hours prior to arrival will result in forfeiture of the entire room deposit.

- Workshops
- Cancellations must be received in writing by Friday, April 11, 2003, for a full refund
(less the $25.00 processing fee). Cancellations will not be processed after April
11, 2003.
